All Huisartsenposten Rijnmond employees are bound by a duty of confidentiality. They handle your data with extreme care. Viewing, modifying or deleting data

You have the right to view, modify or delete your personal data to the extent possible within the law. In addition, you have the right to withdraw any consent you have given to data processing, or to object to the processing of your personal data by Huisartsenposten Rijnmond. You also have the right to data portability. This means that you can submit a request for us to send your personal data that we have in our possession in the form of a computer file to you or another organisation named by you.

You can use this application form (Dutch) to ask to view, modify or delete your data. You must send the completed application Form and a copy of your identification to fg@haprijnmond.nl.
We ask you to provide a copy of your proof of identity to ensure that you are the person making the request to view your data. We do not keep this copy, and we destroy the copy after we have used it to check your identity. To protect your own privacy, you can black out your passport photo, the MRZ (machine readable zone, the strip of numbers at the bottom of the passport), passport number and Citizen Service Number (BSN) in this copy. You can request the transfer of your personal data, withdraw your consent, or object to the processing of your personal data by sending an e-mail to fg@haprijnmond.nl.

We will respond to your request as quickly as possible, and within four weeks.
